This course is designed as a resource to help prepare the pharmacist for the responsibility of monitoring and managing anticoagulation therapy patients and clinics. It is also designed to serve as a starting point to help the pharmacist prepare for the Certified Anticoagulation Care Provider (CACP) exam. This program is approved by the Florida Board of Pharmacy for 18.5 consultant pharmacist recertification hours. Participants who successfully complete the program will also receive a certificate of completion. 

Program Overview

Module 1: Overview of anticoagulant medications

The educational material will provide an overview of medications commonly used in the management of anticoagulation therapy.  Content will specifically cover the history of anticoagulant medications; discuss the clotting pathway; and review the pharmacology of warfarin, Direct Oral Anticoagulants (DOACS), and unfractionated heparin/low-molecular weight heparins. An overview of patient education concepts and pharmacist impact on anticoagulation management will also be reviewed.

Module 2: Disease state management and special populations

The educational material will discuss the antithrombotic/thrombolytic management of commonly encountered disease states requiring antithrombotic/thrombolytic therapy.  Program content will also discuss the management of anticoagulation related to pregnancy and pediatrics.  Learners will have the opportunity to apply the content learned to develop patient-specific individualized treatment plans in the context of the disease states and special populations reviewed.

Module 3: Anticoagulation Complications and Periprocedural Management

The educational material will provide information on the management of complications that may occur with the use of anticoagulant medications, including considerations for management of bleeding, reversal of anticoagulation, and the management of Heparin Induced Thrombocytopenia.  Programmatic content will also discuss the management of anticoagulants in the perioperative setting. Learners will be provided the opportunity to apply the content learned to develop patient-specific individualized treatment plans in the context of simulated clinical scenarios.

Module 4: Operational Components of Anticoagulation Management

The educational material will review the operational and administrative topics related to anticoagulation management.  These topics discussed include the development and design of an anticoagulation clinic, considerations for billing and reimbursement for services, and the use of point of care devices for INR testing.

Schedule of Activities and Requirements for Successful Course Completion

To successfully pass this course the participant must complete the following:

  • View all recorded lectures
  • Complete all learning assessments
  • Complete all case assignments

All program content must be submitted and the participant must achieve a passing score of 80% or more on all course assignments to receive continuing education credit and a certificate of completion.

Credit Reporting

  • The University of Florida College of Pharmacy will report CPE credit to CPE Monitor and CE Broker when applicable. If you would like a paper statement of credit, you may print it from your NABP e-profile.
  • All pharmacists and pharmacy technicians maintain their own license and/or certification and it is their responsibility to follow up with the Office of Continuing Education at the University of Florida if any CE credit discrepancies are found.
  • It is at the University’s discretion to award or deny credit to a late claiming participant (i.e. after 60 days of the participant engaging in a CPE activity) as it is the University’s CE activity and the provider needs to ensure the requirements necessary for credit are successfully completed.
  • The University may request access to CPE Monitor® if the activity in question occurred within the previous 6 months and with an appropriate reason for access, i.e. power outage, etc.
